Évolutions du nombre d’interruptions volontaires de grossesse et du recours aux contraceptifs réversibles de longue durée en Aquitaine suite à la crise de la pilule de 2012

Abstract : Context: several studies have shown that the 2012 pill crisis has resulted in a change in birth control methods in metropolitan France. In addition, the DREES found a 4.7% increase in the total number of medical abortions in France in the PMSI data bank from 2012 to 2013. Objectives: the main objective of this study was to assess the immediate and medium-term impacts of the 2012 pill crisis on the recourse to medical abortion and the use of IUDs and subcutaneous implants in Aquitaine by means of Time Series Analysis. Methods: an observational, longitudinal and retrospective approach was used to analyze time series constructed from the PMSI and Health Insurance performance indicators. In addition, an uncontrolled before-after study was conducted on the PMSI data. The main indicators were: monthly numbers of medical abortions and post-abortion recourses to IUDs and subcutaneous implants in hospitals; and monthly reimbursements of medical abortions and IUDs delivered in non- hospital facilities. Results: the study showed a significant rise in the use of post-abortion IUDs in hospitals by + 40.5% in 2013 as compared to 2012 (p = 0.001). This progression was more pronounced in younger women, with a + 96% increase in the age bracket of 15-19 years old (p = 0.03) and a + 67.8% increase in the age bracket of 20-24 years old (p = 0.0005). The study showed an overall + 8.9% increase in the rate of medical abortions in hospitals in 15-49 year old women (p = 0.036) during the last semester of 2013. This progression concerned the 20-24 year old women (+ 8.7%, p = 0.01) and quite unexpectedly, women over 40 years of age (+ 37.5%, p = 0.02) Conclusion: it can be hypothesised that changes in birth control methods subsequent to the pill crisis contributed to the increase in the number of medical abortions in Aquitaine in 2013. An interrupted time series study based on an ARIMA model could be conducted in order to confirm this hypothesis. So as to avoid the onset of a new pill crisis, better communication strategies about the risks of the pill and means to improve physician practices should be sought.
