Recours aux soins du sujet âgé en milieu isolé en Polynésie Française : déterminants et vécu du soin

Abstract : Introduction: the aim of the study was to describe and evaluate the influence of singular behaviors on the procurement of healthcare services, in order to adapt supply to demand in this population, where there lies a great difference between modernity and tradition, particularly in older generations. Methods: qualitative research based on semi-structured interviews of people above 70 years of age living on 5 different islands. Results: 16 interviews were transcribed. The study shows that health is seen as a status, allowing for work, and illness as aging, as the loss of this ability. Life expectancy isn’t reflective of the subject’s notion of health. Traditional medication is still a deeply embedded part of health behaviors, but we observe a breakdown in the family transfer of knowledge, which is not criticized. Refusal of care is motivated by the acceptance of whatever may happen when that person considers his/herself old enough, or by the fear of invasive procedures. The demand is not on medical supply but on the extent to which it is possible the help of another in everyday life. Finally, the notions of individual and collective responsibility of health have a positive impact on healthy behaviors. Discussion: specific behaviors exist and are linked to culture, geographical isolation and age. They can be somewhat of an obstacle in the understanding between patients and caregiver. They can, however, also be a powerful instrument in order to improve effective healthcare via the promotion of health and allowing the elderly to stay in their own homes through end-of-life care. This research, a first of its kind, opens the way for further investigations on healthcare strategies adapted to culture.
