Score clinique d'obstruction intestinale au cours de la maladie de Crohn (CDOS). Étude de corrélation entre le score et ses composantes et les caractéristiques cliniques et para-cliniques des patients à partir de l’étude CREOLE

Abstract : Introduction: strictures occurs in 50% of patient suffering from Crohn’s Disease (CD) after 20 years of evolution. A clinical score, (CDOS) measuring obstructive’s symptom, have been developed by a the Groupe d’étude des Affections Inflammatoires du tube Digestif (GETAID). Obstructive symptoms, have been prospectively reported, with a global estimation (EVA) of obstruction made by physician going from 0 (no obstructive symptoms) to 100 (maximal obstructive symptoms) over a 8 weeks periods. The aim of this study is to look for the correlation of CDOS and EVA. Patient and method: it is an ancillary study of the Creole Study. Link between CDOS and EVA was studied by simple linear regression. Other CDOS, made from the original and a new CDSO called CDOS2 were evaluated by multiple regression. CDOS has been validated in our cohort by Bootstrap and then compared to clinical, biological and RMI data by simple linear regression. Results: ninety-seven, 90, 85, and 77 patients have been respectively included at weeks 0,8,16 and 24. Correlation between CDOS and EVA was poor (r=0,564 ; p<0,001). Alternatives models of CDOS were generated which can improve a little this correlation (r= 0,584 et r=576 ; p<0,001). A new score (CDOS2) has been created by multiple linear regression. CDOS has a better correlation to EVA than CDOS (r= 0,66 ; p<0,001). Internal validation by bootstrap shows a good correlation (r=0,631 ; IC95% [0,541 ; 0,704] ; p<0,001). Conclusion: correlation between CDOS and EVA was poor. Our result allowed the creation of a new CDOS well correlated to EVA, with internal validation, which one needs to be validated in an independent cohort.
