Quantification des trabéculations ventriculaires gauches chez des patients atteints de cardiomyopathie hypertrophique, en comparaison à des sujets sains

Abstract : Introduction: Hypertrabeculation is the main symptom of LVNC (Left Ventricle Non Compaction), however LV hypertrabeculation might be a phenotype trait overlapping in other cardiomyopathies, such as hypertrophic cardiomyopathy (HCM). We used a semi-automated method to quantify trabeculated mass across the LV in a cohort of HCM patients, in comparison with healthy subjects. Method: HCM patients were recruited from January 2015 to December 2015 in our institution. Healthy volunteers were prospectively recruited to match the patients regarding baseline characteristics. For each subject trabeculated (T), compacted (C) and papillary muscles (PM) masses were evaluated with a semi-automated software. The T/C and PMT/C ratios were then computed. A subgroup analysis was performed comparing Patients with severe LV hypertrophy (LVH+, C mass>85g/m²), Patients with moderate hypertrophy (LVH-, C mass<85g/m²) and Controls. Results: 75 subjects were recruited in each group (37 LVH+ and 38 LVH- patients in each subgroup). T/C and PMT/C ratios differed significantly between Patients and Controls (13.4±3.1% versus 9.9±2.6%, p<0.001 and 17.8±3.2%, versus 13.9±2.9% p<0.001). These differences were higher comparing LVH- subgroup and Controls (LVH-: 13.9±3.4% and 18.3±3.5%; Controls : 9.9±2.6% and 13.9±2.9, p<0.001). Conclusion: T/C and PMT/C ratios show that hypertrabeculation could be a phenotypic trait of HCM, especially for patients with moderate hypertrophy.
