Rôle du chirurgien dentiste dans la prise en charge d'un syndrome de Cowden : revue de la littérature et illustration par un cas clinique

Abstract : Cowden Syndrome (CS) was first described in 1963 by Lloyd and Dennis. It's an uncommon autosomal dominant disease that is characterized by the development of multiple hamartome and an increased risk of malignancy, most commonly breast, thyroid, endometrial, colon and renal cell carcinomas. Germline PTEN mutations in CS were first found in 1997. First clinical manifestations often occur in the second to third decade. With its varied phenotypic expression, this disease is generally unknown and many patients are diagnosed at a late stage. The prevalence of PTEN mutations could be around 1 / 200 000 in the world. Since 1996, The National Comprehensive Cancer Network annually updates diagnostic criteria for Cowden Syndrom. Mucocutaneous hamartomes are the most common lesions and mainly include oral mucosal papillomatous lesions in 100 % of CS (Starink 1986). We report a case of a 101 years old Caucasian female with three oral papillomatous lesions including a giant one. Investigations revealed others typical clinical neoplasics lesions of CS. Therefore, the odontologist has a major part to take in the early detection of those lesions, leading to an early detection of the syndrome and malignancy prevention.
