Application du garrot tourniquet en traumatologie civile : revue systématique de la littérature

Abstract : Introduction: in the context of forward combat casualty care, the application of the tourniquet is effective to stop extremity hemorrhages. However, the use of tourniquet in civilian trauma setting is low. The aim of this systematic review was to analyze the evidence-based medical literature in order to define the use of tourniquet in civilian trauma management of extremity hemorrhages. Methods: analysis of all studies published until 01/10/2016 on the Embase, Medline and Opengrey databases. Included studies concerned the use of tourniquets in civilian trauma setting. The quality of the articles was evaluated using the PRISMA criteria and the STROBE statement. Results: among 355 studies identified, 24 were included. The global level of evidence was low. 3028 applications of tourniquets were reported, most of them involving the Combat Application Tourniquet CAT®. Reported hemorrhages were almost exclusively traumatic, occurring in young men (27-44 years). The effectiveness rate of tourniquets varied between 78% and 100%. The complication rate associated with the application of tourniquet in elderlies or patients with comorbidities was low. Finally, caregivers reported a common fear of adverse effects, while reported complications were rare (<2%). Conclusion: in this systematic review, tourniquet was an effective tool for the management of extremity hemorrhages in civilian trauma, associated with few complications. Further larger studies and development of dedicated trainings are still required to definitively deploy the use of tourniquet in dedicated guidelines.
