Facteurs de non conformité des prescriptions par inhibiteurs de la pompe à protons en médecine générale : une étude comparative rétrospective chez une série de patients de plus de 75 ans hospitalisés

Abstract : Context: Many studies on the subject of proton pump inhibitors (PPI) have shown a majority of non appropriate use and emerging adverse effects, with particular concern among older adults. Objective The aim of the study was to determine the factors of non-indicated prescriptions of PPI’s in general practice among patients aged over 75 years. Methods: This was a retrospective and comparative chart review from acute geriatric medicine inpatients at the hospital of Saint-Malo, conducted from 1st June to 31st July 2015, on patients who had a current prescription for a PPI. Results: 58 patients and their 42 general practitioners were included in the study. 67,2% of prescriptions were non-appropriate. They were significantly associated with no performance of an upper endoscopy (p = 0,0000002), a frequency of office visit equal to higher than once per month (p = 0,0001), and no attempt to stop the PPI’s treatment (p = 0,021). There was a significant association with the therapy’s indication (p = 0,00001). Non-appropriate therapies were mainly prescribed for digestive injury prevention among patients treated with antiaggregants (35,9%). 31 to 66,7% of doctors prescribed PPI’s without valid indication. 9,5 to 45,2% of the general practitioners were aware of the adverse effects of PPI’s. Conclusion: Our study shows an insufficient knowledge about PII’s recommendations and adverse effects. The focus should be on continuing medical education and professional practice assessment.
