Prise en charge pharmacologique de l'agitation psychiatrique chez l'adulte : une revue de la littérature

Abstract : Background: psychiatric agitation is a common situation in emergency departments. It requires rapid management to prevent any danger to the patient or professionals present on site. Method: we studied the international literature on Pubmed and Cochrane databases over the period 1973 to 2017 to answer the following questions: what is the state of current knowledge on the pharmacological management of psychiatric agitation? Which molecules have proven their effectiveness and what are their risks urgently? What pharmacological strategies are validated at the international level? Results: benzodiazepines are no longer recommended as monotherapy in agitation induced by a serious psychiatric disorder. They should only be used in combination with a first-generation antipsychotic in this indication. They remain the first-line treatment in the management of alcohol withdrawal. First-generation antipsychotics are gradually being supplanted by second-generation antipsychotics. Phenothiazines are no longer recommended, only Haloperidol is indicated as first-line treatment for alcohol-induced agitation and second-line therapy in combination with benzodiazepine for the management of alcohol-induced agitation emergency, a serious psychiatric disorder. Second-generation antipsychotics with Olanzapine as a leader are first-line therapies in many indications as monotherapy. Conclusion: improving the management of the psychiatric emergency makes it possible to find alternatives to restraint and contributes to a better therapeutic alliance afterwards.
