Quelles informations sont à disposition d’un professionnel de santé, en cas de soin non programmé d’une personne âgée

Abstract : Introduction. Elderly people request more and more unscheduled care, both in ambulatory medicine and emergency structures. However, the management of these patients requires an overall evaluation, which is often difficult. Objectives. To analyze the information and transmission vectors available to a doctor, in an unprogrammed situation, with a patient over 75. To investigate the correlation between this information and the quality of care. Material and methods. Multicenter observational study, by self-administered questionnaires, in France. Were included all situations of unscheduled care, involving an elderly over 75, and a physician, with specific training in general medicine, who was the first medical responder and who didn’t know the patient before. Results. 174 questionnaires were included. 36,2 % of doctors reported that their care was not satisfactory. Patient's medical history, treatment and autonomy were the three most frequently recovered information. The results of paraclinical tests, the medical history and the autonomy were significantly associated with a good management. Patients who consulted a doctor with a medical report or for cardiovascular motive also benefited from better care. Conclusion. A closer medical follow-up, the immediate delivery of the medical reports and the systematic inclusion of paraclinical tests results in the reports, are some proposals that could be studied for a best management of elderly, in the expectation of an national uniform computerized vector.
