Impact des stéatopathies métaboliques sur les évènements cardiovasculaires dans la broncho-pneumopathie chronique obstructive

Abstract : Rationale: Chronic Obstructive Pulmonary Disease (COPD) and Non-Alcoholic Fatty Liver Disease (NAFLD) share intermediate mechanisms, independently increase cardiovascular risk and their coexistence might accelerate the occurrence of cardiovascular events. Methods: The relationship between NAFLD, cardiovascular events, deaths, and acute exacerbations of COPD was assessed in a prospective cohort of COPD patients with a 5-year follow-up. Non-invasive tests (fibromax®) evaluated the presence of steatosis, non-alcoholic steatohepatitis (NASH) and liver fibrosis on inclusion. Our objective was to evaluate the impact of steatopathy on the occurrence of cardiovascular event, death and exacerbations of COPD. Results: 111 COPD patients were included, 75% exhibited NAFLD with a prevalence of steatosis, NASH and fibrosis of 41%, 37% and 61% respectively. During the 5-year follow-up, 56 patients had at least one acute exacerbation of COPD, 34 had at least one cardiovascular event and 7 died. Patients with NAFLD experienced more cardiovascular events (37% vs. 11%, p = 0.01). In univariate and multivariate analysis, fibrosis was the only liver disease associated with the occurrence of cardiovascular event or death (HR = 2.94, [1.18, 7.33]). NAFLD was not associated with an increase of COPD exacerbations. Conclusion: Early evaluation of metabolic steatopathy, especially at the stage of fibrosis, could improve the recognition and prevention of cardiovascular risk in COPD patients.
