Les travaux de thèse en médecine générale, une discipline centrée patient, un travail centré médecin

Abstract : Introduction: The general medicine research has known an exponential development since the last century. The Pouvourville (2009) report mentioned that it is a necessary to rely on medical proof to improve the primary care. As a result, it is easy to imagine that the patient is the main purpose for the research. However, some previous works have pointed that it is not the case. Methods: Retrospective descriptive study about TESMG from the 01/01/2009 to 12/31/2015. The reading of abstracts for each dissertation by two of the authors allowed a double collect of the following information: medical speciality of the dissertation manager, the purpose (the patient or not), methodology, field concern (the physician (Mg), Mg + miscellaneous, Mg + patient, patients, patient + miscellaneous, miscellaneous, Mg + patients + miscellaneous), the writing in article dissertation and the price proposals. In case on unconformity data, a consensus has been established in the presence of a third person. An adjusted model has been used to modelize the probability that a dissertation doesn't take into consideration the patient. Results: 48% of the dissertation didn't include patient and 25% wasn't about the MG field. However, the dissertation targeting the patient was gradually less forgotten since 2010 and more focus on patient by far since 2013 vs 2009 (OR = 0,512 in 2014, OR = 0,375 in 2015). In the case that the dissertation manager was a generalist physician, dissertations were less focus on patient (46,0 % vs 61,9% (p<0,01%)). Conclusion: In the past, MG dissertations have been focus on the physician but since 2013, this trend is reversed. Solutions are there. A common specific reform of TCEM about the writing modality for MG dissertation could contribute to generalize a research focus on patient.
