Syndrome douloureux chronique dysfonctionnel versus fibromyalgie : étude comparative des critères cliniques

Abstract : Fibromyalgia is a common pathology. Its diagnosis is regularly re-evaluated and improved. However, patients suffering from dysfunctional chronic pain do not meet the criteria of this pathology and remain undiagnosed. The objectives of this study were to describe the epidemiological and semiological characteristics of these patients in order to define them better and then, by comparing them with fibromyalgia patients, may help to make a diagnosis. The data were extracted from patient files who were examined at the Percy Pain Consultation Center between 2012 and 2017. The study population included 26 dysfunctional chronic pain patients and 54 fibromyalgia patients. Dysfunctional chronic pain patients had an average of 44.19 years, 54% of whom were women. The duration of their symptoms was similar to that of fibromyalgia patients with an average of 6.02 years.The two groups of patients had similarities in pain intensity and resonance. Their differences rested on the extent of their pain. Dysfunctional chronic pain patients had asymmetric involvement (85%) with an average of 37% [17-67] of painful body surface area. Fibromyalgia patients had bilateral (91%) and symmetrical (58%), fluctuating (89%), affecting on average 68% [28-100] of the body surface area. Fibromyalgia is therefore a severe form of dysfunctional chronic pain, the aggravating factors of which remain to be determined.
