Réhospitalisations à sept jours des patients admis depuis les urgences pour un motif médical : prévalence, caractéristiques et facteurs associés

Abstract : Seven-day readmission rate (7DR) is a quality of care indicator for the healthcare provided by hospitals. The main objective of this study is to describe the index stays (IS), the related 7DR, and search for risk factors of early readmissions. A retrospective observational study was conducted at Grenoble Alpes teaching hospital in 2017. The patients admitted via the emergency department for medical conditions were included when they met the criteria of the ministerial indicator. The medical and demographic data of the patients and the calendar and organizational characteristics of the stays were collected. The planned or unplanned character of the 7DR was sought. Seven thousand and twenty IS were included, of which 355 (5.06%) were followed by a 7DR. One hundred and fourteen (32%) were planned. Risk factors for an unplanned 7DR were: an age of 75 years or older (57.26% vs. 44.59%, p<0.01), Charlson comorbidity score of 4 or greater (16.60% vs. 10.34%, p< 0.01) and 3 or more stays in the previous year in the establishment (15.35% vs. 8.30%, p<0.01). IS followed by 7DR had a statistically longer hospital length of stay (8.08 vs. 7.26 days, p=0.05) than those not followed by 7DR. Patients with an IS for a stroke were statistically less readmitted; while IS for an ileus were at risk of unplanned 7DR (3.73% vs. 1.60%, p<0.05). 7DR are rare events, but medical and demographic characteristics can help identify at risk patients. The ministerial 7DR indicator does not distinguish between planned or unplanned 7DR.
