Adhésion médicamenteuse des patients sous antidépresseurs : quels freins ? Quels leviers ? Analyse sur un échantillon de 117 patients en médecine de ville

Abstract : Target: The purpose of this study was to suggest the need for an inventory of a drug adhesion to antidepressant treatments in an urban medical environment and to test the use of a measurement tool that would enable to assess the patient's adhesion (the self-administered questionnaire QUILAM). Approach: A total of 117 subjects, spanning in age from 18 to 91 years, having had antidepressants between September 1st, 2016 and January 31st, 2017 filled out a survey allowing the quantitative (Morisky-Green scale) as well as the qualitative (QUILAM questionnary) assessment relative to medication adhesion. Results: The majority of them were women (69%), aged 55 in average, with an antidepressant SSRIs (71%). In 74 per cent of cases, the general practitioner had prescribed it. The use of the tailored Morisky-Green questionnaire enabled to demonstrate that about 60% (63%) of the patients showed a lack of compliance. The use of the QUILAM questionnaire showed that 42% of the patients were pinpointed as at adhesion risk for at least one dimension of the drug adhesion. The risk adhesion related mainly to the relationship between the patient and the care system (54%) and also to the general representation of the treatments (28%). The correlation between both questionnaires was tested and estimated at 53%. Conclusion: To date, the patient's adhesion to antidepressant drugs within an urban environment is far from being optimal. An intervention at different levels would allow to increase the patient's care adhesion. The QUILAM questionnaire is a promising tool in the identification of obstacles to medication adhesion.
