Caractéristiques du syndrome chronique de loge antéro-externe de jambe chez les skieurs nordiques de haut niveau

Abstract : Introduction: The frequency of chronic exertional anterior and lateral compartment syndrome (CECS) of the leg in the cross country skier population is unknown. The main purpose of this study was to evaluate the prevalence and attack rate of this syndrome in the biathlete and cross country elite skiers. The second objective was to analyze the functional impact of CECS, to spot the contributing factors and to analyze surgery results. Method: A retrospective study was carried on 294 skiers of the French national team from the winter season 1994/1995 to 2013/2014. Mailing, texting and medical record analyze have permitted to list the cases. The patients presenting a CECS have been contacted by phone to answer a questionnaire. Results: 18 patients presenting a CECS have been counted that was a 6,1% prevalence and an attack rate of 13 cases for 1000 skiers season. The prevalence in the cross country skiers tend to be less than in the biathletes (p=0,08). The mean age of apparition of CECS was 18 years old. 100% had very severe difficulties. The roller ski skating at high intensity and skating on technical snow triggered pain in all cases. Before to develop the CECS, 24% changed material and 47% passed in a team of higher level. The mean age at surgery was 19,5 years old with only 1 failure. 94% were satisfied, 88% resumed practice at the same or higher level. 35% went back to skiing in the month following surgery. Conclusion: The CECS was frequent in the cross country skier population and occurs early. Cross country skiing was likely to be involved in solicitation of the anterior and lateral compartment muscles. The results of surgery were good allowing to resume at high level.
