Améliorer la qualité et la continuité de la prise en charge médicamenteuse : du suivi pharmaceutique en hospitalisation psychiatrique à la transmission de la lettre de conciliation de sortie au médecin traitant

Abstract : Since 2015, all admitted patients hospitalized in our psychiatric hospital received completed medication report (medication reconciliation, patient interview, pharmaceutical analysis). The goal is to deploy the medication reconciliation at discharge activity to secure the patient's drug management when he leaves hospital. A pharmaceutical care was created and integrated into the patient file. This allows to continuously collect, synthesize and quickly integrate pharmaceutical informations into the medication reconciliation letter of discharge before its sending to the general practitioner by secure messaging. In our study, only at-risk patients (age ≥ 65 years, 1st hospitalization, anticoagulant therapy, insulin, immunosuppressive drugs) are eligible for the medication reconciliation at discharge. The activity was implemented for 10 months in 2 admission services. Of the 442 admissions, 92 of 128 eligible patients benefited of a medication reconciliation at discharge. Pharmaceutical interventions to optimize the discharge prescription were accepted at 83 %. For 71 letters, at least one pharmaceutical observation (side effects, informations about the dosage form of a drug) and 81 had at least one summary of medical monitoring which must be continued by the general practitioner (cardio-metabolic monitoring, INR, TSH). We have been sent 90% by secure messaging. A survey of general practitioner showed their full satisfaction. This period has made the process structured and efficient. Our desire is now to consolidate this activity in these two admission units and extend it to other services.
