Évaluation des variations des capacités attentionnelles chez les soignants des urgences au cours de leur plage de travail du Centre Hospitalier Universitaire de Bordeaux site Pellegrin

Abstract : HCPs ( Health Care Professionals) shift work in Emergency wards can lead to errors due to changes in vigilance levels over the working time. The aim of this study was to describe those changes and assess whether or not they are influenced by the individuals pace of work (7:30, 10h or 12h). The primary objective was to assess the changes in individuals psychomotor skills and vigilance between the beginning and the end of their working time. The secondary objective was to compare those changes according to the pace of work . This is a prospective & descriptive monocentric study conducted in Winter 2017 using self-administered questionnaires completed before and after work, over two different working times. The test used was the Trail Making Test , which describes stress and fatigue with scales ranging from 0 to 10. Out of 110 distributed questionnaires 30 were completed and sent back to the investigator. The median overall TMT scores for condition 3 were significantly lower at the end of the working period (-0.5, p = 0.038). This difference was found even in individuals working for 7:30 hours (-2, p = 0.027). Overall, the study population were more stressed and tired at the end of their working time. Those individuals working for 12 hours in a row rated harder working conditions than those working for 7h30 or 10 hours . Due to a limited number of respondents, this study was not powerful enough to provide an accurate assessment of variations. Larger sample studies associated with more sensitive tests would help confirm those first results and better identify the factors influencing changes in vigilance levels.
