Préparation de dose à administrer automatisée : les limites en EHPAD de la méthode déconditionnement-reconditionnement

Abstract : Dose administration aids is a common practice in a dependent old people’s home. This work is under pharmaceutical responsibility and automatic machines are available to do it. In Aunay-Bayeux Hospital, a few years ago, the choice has been made to a deconditioning – repackaging automaton. This why we want to reevaluate this practice. This automaton has many advantages as an obligation of computing prescriptions, an administration tracing, a faster production and frees more time for nurses. Furthermore, the cost is affordable. These advantages should be tempered with disadvantages as a poor reliability with errors detected in pharmacy and health services. It produces many wastes. The depackaging raise the question of the stability of the medicinal product. This practice poses a legal problem. Finally, this automaton works with only half of prescriptions for different reasons as an incompatibility of drugs with the automaton, logistics issues, a risk of cross reactions or legal reasons. An over-conditioning automaton allows to resolve many problems but it has a significant cost. The deconditioning-repackaging automaton remains a sensible solution. It’s important to keep in mind these limits to optimize its use.
