État des lieux de la prise en charge de la mort inattendue du nourrisson en Guyane française : étude rétrospective de 2006 à 2016

Abstract : Sudden unexpected infant death (SUID) is a public health problem for which french recommandations in 2007 can’t be applied in French Guiana. The aim of this study was to describe how these children were taking care of and to purpose guidelines adapted for Guianese context. Methods : We conducted a retrospective, observational and multi-center study including all cases of SUID in French Guiana during 11 years. Pre-hospital, hospital and medicolegal care data were collected. Results :Fifty five files were included, 47,3% from Cayenne’s island, 29,1% from the other coast’s towns and 23,6% from inside’s towns. Median age was 125 days. Forty two cases (76,4%) died at home. Emergency medical service intervened in 45,5% of cases. Resuscitation was carried out in 60% of cases. Blood samples were taken in 47,1% of cases. All body CT scan was performed in 35,3% of cases and a medico-legal autopsy in 25,5% of cases. No medical autopsy was performed. Conclusion : There is no reference’s center for SUID in French Guiana and it makes SUID management more complicated. Not enough recommended tests are done, especially in inside’s towns because of the lack of technical means and difficulties of accessibility. Improvements have to be made and discuss with all concerned actors.
