La souffrance myocardique (quel pronostic pour quels patients ?)

Abstract : Background: Myocardial injury (MI) has been defined in 2017 as an elevation of seric troponin, without any sign or necrosis, neither ischemia. This entity includes multiple aetiologies, and it may be difficult to make a difference between myocardial injury and type-2 myocardial infarction. We aimed to define MI’s one-year mortality, and the characteristics of the concerned population. Methods: A retrospective, monocentric, observational study took place in the Emergency department of Marseille’s Hôpital Nord, among all patients having a positive troponin test. All patients having any criteria of myocardial infarction, or any other identified cause leading to a false troponin elevation, were excluded. Results: Among 531 patients identified in the Emergency department, 280 were classified in the « myocardial injury » category. The mortality rate was 50.4% after one year, and the most common aetiologies were heart failure (35% [29% - 41%]) and sepsis (33.9% [28% - 39%]). Conclusion: With a very poor outcome after one year (more than half of the patient being dead), myocardial injury, without any distinction of its aetiology, shouldn’t be underestimated in the emergency department.
