Prise en charge et devenir des traumatismes sportifs du genou aux urgences du CHU de Caen

Abstract : Introduction: sport provides lots of health benefits. However, sports traumas drive many visits in emergency departments. We have studied the management of sport-related knee injuries in the emergency department of CHU de Caen, and the patients’ follow-up. Methods: we conducted a retrospective cohort study about 171 patients presenting sport-related knee injuries, admitted to the emergency department of CHU de Caen, from March 1st, 2017 to February 28th, 2018. Results and discussion: the average age of patients was 25.2 years. Most of them were men (73.7%) and football was the most incriminated sport, causing 42.7% of injuries. Patients were often referred to orthopaedics (45%), whereas competitors were more referred to sports medicine department. Physiotherapy prescription at emergency department, which concerned only 9.9% of patients, would improve the management. Of the 41 anterior cruciate ligament lesions diagnosed during the follow-up, only 3 had already been suspected at the emergency department. Competitors were more often operated in private hospitals. Three months after the trauma, more than 40% of patients could not return to play. Conclusion: sport-related knee injuries are common. The diagnosis at emergency department is difficult. An early reassessment in sport medicine department could improve diagnosis and therapeutic management, in order to allow return-to-play in better conditions.
