Évaluation des pratiques professionnelles de la prise en charge des embolies pulmonaires en médecine de ville

Abstract : Pulmonary embolism is a real public health matter, given its frequent, multifactorial and sometimes serious nature, especially in its acute presentation, and a longer-term morbidity and mortality vector. Thus, this descriptive prospective observational study evaluated GPs' adherence to the ESC 2014 recommendations on VTE in their daily practice. The data collection was done using a computerized questionnaire through "Google Forms". The response rate was 23%, with a sample that was not representative of the general population at the national level. The results of this study confirm the existence of a discrepancy between the guideline and its application on the outpatient side. Indeed, only 1 practitioner out of 2 declares to know the recommendations in force, 2/3 use the scores of aid to the diagnosis as well as the dosage of the D Dimers in a systematic way. Only 29% of the sample say they know the sPESI score, when half of them use direct oral anticoagulation. In the future, we can imagine an ambulatory care network allowing external management of some of these pathologies, which can lead to an improvement in the overall cost of care, a better understanding and an increased adherence of the patient to his treatment and his pathology. An American study called HoTPE is currently under way.
