Impact de la tératospermie sur les résultats d’insémination intra-utérine : étude de 791 tentatives

Abstract : Introduction: Intrauterine insemination is one of the oldest reproductive techniques. In 2015, 50,714 homologous inseminations were realized in France with a result of 12.18% of pregnancies. This rate being low, it seemed important to know the predictive factors of success in order to improve this pregnancy rate. The outcome was to determine the impact of a spermatic abnormality, i.e. the teratozoospermia, on the homologous intrauterine insemination pregnancy rates. Materials and Methods: the study was retrospective, comparative and monocentric. It concerned intrauterine insemination attempts carried out between January 1st, 2009 and December 12th, 2017 in the department of Medicine and Biology of Reproduction, Cytogenetics and CECOS of the University Hospital of Amiens. The analysis provided the comparison between two groups: group 1, “couple whose man has no spermatic anomaly” and group 2 “couple whose man has isolated teratozoospermia”. The principal criterion was the pregnancy rate obtained by attempt in each of these groups. Results: Seven hundred and ninety-one attempts were selected, including 591 in group 1 and 194 in group 2. The two populations were strictly comparable. No significant difference was found for the pregnancy rate per attempts (14.57% in group 1 versus 15.98% in group 2, p=0.63). Discussion: The literature was contradictory on the subject. Our results were consistent with recent research investigating the impact of isolated teratozoospermia on insemination outcomes. The number of mobile spermatozoa inseminated seemed to be a more favorable predictive factor of pregnancy: the presence of a large number of mobile spermatozoa would compensate the teratozoospermia, even severe. Conclusion : Our study did not reveal the impact of teratozoospermia on the results of intrauterine inseminations. Further investigations would be needed to find a definitive answer.
