Motivations et caractéristiques de la population de cabinets de médecine générale spécialisés en soins non programmés, en dehors de leurs activités de permanence de soin, à La Réunion

Abstract : Introduction: rising demand for emergency and non-programmed treatments in private medical care has led to the development and the expansion of these medical centres in France. In Reunion Island, some medical centres guarantee permanent ambulatory care as well as emergency care during General Practitioners’(GP) consulting hours. Objectives: define the patients’ socio-demographic characteristics during this specific time slot, examine the reasons for accessing or choosing emergency or non-programmed medical centres rather than General Practitioners’ appointments. Material and Methods: for three weeks, using digital tablets, a survey was carried out on patients in the waiting rooms of two emergency and non-programmed medical centres. Results: results from 1085 patients showed that, 35% of patients came for paediatrics, 58% were between 16 and 54 years old, only 7.3% were over 55 years old. 57.1% were working people. Only 8.5% of them were sent by health professionals, the others came voluntarily. 60% of patients came for somatic reasons and 15% of them came with traumas. In 50% of cases, their General Practitioner was not available. 52.3% of patients came for an emergency. 7% of patients came because they were not satisfied with their General Practitioner or wanted another point of view. The approved health care pathway was respected in 69% of cases. The average of doctors’ appointments was 4.85 per year per patient. Conclusion: the patients choosing to access emergency and non-programmed care centres were often young and in working-age. The patients’ perceptions around accessibility and availability of GP’s appointments led them to emergency and non-programmed centres.
