Évaluation de l'impact de l'utilisation d'un carnet de surveillance de poids sur les hospitalisations pour décompensation cardiaque dans une population gériatrique

Abstract : Objectives: the aim of these study was the evaluation of a weight-control logbook for the chronic heart failure patient Material and method: this was a prospective, comparative, interventional, randomized, non-drug, single-blind study. The study was carried out from 1st January 2017 to 20th June 2018. Each patient is included for a duration of 6 months. The population contained 70 patients, over 70 years old, hospitalized for acute cardiac decompensation and carrier of chronic heart failure. The primary endpoint was to compare rehospitalization for acute heart failure at 6 months Secondary criteria included identifying risk factors for rehospitalization at 1, 3 and 6 months and comparing rehospitalization at one month and three months. Results: results on the primary endpoint included 12 hospitalizations for the log group versus 18 hospitalizations for the control group (p = 0.13). The presence of spatio-temporal orientation disorders was a risk factor for rehospitalization at 1 month, RR = 4.2 (p = 0.003) and at 3 months RR = 14 (p = 0.006). Maintaining autonomy for dressing was a protective factor for rehospitalization at 1 month, RR = 0.07 (p = 0.005) while autonomy for hygiene, RR = 0.10 (p = 0.04), and going to WC RR = 0.18 (p = 0.01), as well as the presence of a nurse, RR = 0.13 (p = 0.03), were protective factors at 3 months. Conclusion: there was no significant difference regarding rehospitalization between the two groups. Guidance disorders were risk factors for rehospitalization, while the preservation of functional autonomy and the presence of a nurse was a protective factor. A better integration of a nurse in the follow-up of these chronic patients seems to be a way to explore to improve their care.
