Comparaison entre un cathéter péri nerveux fémoral et une attelle réfrigérée Game Ready sur la fonction quadricipitale en postopératoire des prothèses totales de genoux

Abstract : Introduction: the purpose of this before / after study was to compare functional recovery after total knee arthroplasty based on two selected modes of analgesia. One group benefited from a postoperative femoral catheter and the other from the refrigerated intermittent compression. Research Hypothesis: post-opérative intensive physiotherapy would provide at least 35% improvement in active extension after passive flexion (measured in degrees) on day 2 compared with the placement of a postoperative peri-femoral catheter. Material and methods: 51 consecutive patients were included, all of whom had preoperative adductor canal block involvement and intraoperative joint infiltration. The first group of 27 patients benefited postoperatively from the placement of a femoral catheter with a PCRA for three days. The second group benefited from the installation of an intermittent and refrigerated pneumatic compression device (Game Ready R). It was collected postoperatively as a primary endpoint for active extension after passive flexion on day 2 to evaluate functional recovery. Results: the median range of active extension after passive knee flexion was 85 degrees {55-90} in the Game Ready group, and was 80 degrees {45-90} in the BFc group. Nevertheless, no significant difference could be found (p = 0.66). Discussion: this study, not show any difference in terms of knee angulation at day2, which would not show a functional superiority of one device over the other. . No significant differences were found in terms of pain at Day1, Day2 or Day3. The observation of the results of EVA could lead us to abandon the installation of peri-nerve catheter because of the practical and economic side of the device Game ready.
