Évaluation de l'état de santé perçu par le patient âgé en SSR à la veille de son retour à domicile

Abstract : For several years now, the average age of the French population has been increasing, and the use of hospitalization, follow-up care and rehabilitation services is progressing in line with this increase. Many studies have shown that self-perceived health assessment is a strong predictor of various aspects of well-being, functional abilities and also mortality. All of these criteria meet the needs of the geriatric patient profile. Few scales of evaluation have been validated on this subject, they often demonstrate a feeling of quality of life in the present moment and were realized several years ago. The objective of this study is to highlight the factors that have a direct or associated influence on the state of health perceived by elderly patients at the end of a geriatric rehabilitation stay. This could help to guide their care, identify the criteria for aging well and optimize their rehabilitation. It is an observational, quantitative, cross-sectional and monocentric study carried out in the geriatric rehabilitation clinic of South Phoceanne. We included a total of 87 patients over a 10-week period after verification of inclusion criteria and consent authorization for each patient. The procedure was conducted in the form of an individual interview, using a questionnaire based on bibliographic references. Results: 20 patients considered themselves in poor health, 47 in good health and 20 in very good health. Appear as factors related to a perceived poor health status, an isolated family situation, a low socio-economic level, some medical history, old smoking, polypharmacy, feeling of diminished physical abilities, recent hospitalization of more than a month , the absence of a future project and the feeling of dissatisfaction with the current life. We note that for our patients to maintain autonomy and to have a good quality of environment is necessary in order to age in good conditions. Conclusion: Some factors are related to the health status perceived by patients, an early identification of these would guide the management of most-at-risk patients, for example to day rehabilitation to optimize their rehabilitation.
