Points de rupture dans le parcours de soins coordonnés

Abstract : The health care path in our health care system was set up in 2004. The general practitioner was given a central part in the patient's follow up the coordination of his cares and the management of his medical file. Our study aims at collecting general practitioners' opinions about it, thanks to a nominal group technique, allowing us to rank the issues they deal with in the field. We made a qualitative analysis in 2017 in Hyeres and Gap. The general practitioners involved were volunteers. The main disruptions pointed by general practitioners are the lack of interprofessional coordination, administrative tasks, the quality of the doctor-patient relationship, the deficit in communication tools or logistics malfunctions, and care inaccessibility or unavailability. Were underlined next the difficulties linked to prescriptions, the lack of accessible shared medical files, the problems met in preventive care and also the system's contradictions. To illustrate these results, we have made an online questionnaire that was filled by URPS PACA elected general practitioners and by course supervisors from the university. We have also noted the satisfaction rate concerning the coordinated cares current process. Our study allowed us to identify and rank the failures in the coordinated cares path that general practitioners have to deal with everyday. Improving those weaknesses could make the practice of medicine more attractive and would enable us to optimize the patient's care.
