La prise en charge antalgique durant l’interruption volontaire de grossesse médicamenteuse en ville chez les professionnels de santé métropolitains

Abstract : Pelvic pain is recognized as the major side effect of drug-induced abortion. The HAS in 2010 began to detail this analgesic treatment and since 2016, the CNGOF has specified this protocol. We conducted a descriptive multi-center study in metropolitan France between October 2018 and March 2019, whose main objective was to analyze the analgesic management of drug-induced at home abortion up to 7 SA. 393 answers were collected, representing approximately ¼ of the source population, including general practitioners, medical and obstetric gynecologists and midwives. In any population, 66.4% of the analgesic protocols were valid. The Phloroglucinol response was checked by 28.1% of the professionals, and 7.7% did not check the systematization or the treatment of NSAIDs. 29% did not prescribe NSAIDs in the systematic and 38.2% did not recommend the systematic and non-systematic treatment of Tier 2 analgesics. The comparative study of the analgesic protocol according to the personal characteristics showed that the professionals who performed more abortion per year (p-value = 0.0001), who were older (p-value = 0.004), or who were employed by a planning center (p-value = 0.02), had a better compliance rate analgesic protocol. A still considerable number of professionals do not prescribe to their patients an analgesic prescription validated by evidence-based medicine, and continue to prescribe Phloroglucinol and not Ibuprofen.
