Intérêt d'une batterie informatisée pour le diagnostic et la classification des aphasies primaires progressives

Abstract : Although the definition of primary progressive aphasia (PPA) has evolved and a classification of subtypes is created, their diagnosis is still late and classification is sometimes difficult. In order to make an early diagnosis, classification and by extension therapy, we developed a computerized battery. It is based on the classification tree of Vandenberghe (2016) and includes six tests judged as the most discriminating and mainly from the GREMOTS. We compared the results (score and time) between PPA patients and control subjects as well as between PPA subtypes to assess its diagnostic and classificatory interest. We also analyzed the errors produces specifically in each subtype. The overall results highlight a significant difference between the pathological and control population, which demonstrates the diagnostic interest of the battery. As expected, naming is dropped in all APP, praxies are disturbed only in APPnf. Sentence production and photo-word checking are difficult in several subtypes and more in the expected APP. However, the repetition is not significantly different in APPl. Subtype specific errors have been found in some tests and can help to characterize the profile of patients. Given the size of our sample, classificatory efficacy has to be analyzed on a larger number of subjects. The severity of the patients is to be taken into account because in our study it contributes to the inadequacy of profiles with the classification. Finally, some elements of the classification and the Vandenberghe tree may be questionable.
