Impact des chirurgies orthognatiques sur l'étiopathogénie des dysfonctionnements temporomandibulaires : mécanismes et prise en charge du patient

Abstract : By mobilizing the bone bases to cure dysmorphisms, orthognathic surgeries bring important functional modifications to the manducatory apparatus. Among others, these modifications concern the forces applied on the temporomandibular joints and solicit their adaptive capacities. This type of surgery is famous for its positive impact but like every other therapeutics, it can fail. Most of the time it brings an improvement to the articular condition but a significant proportion of worsening and even creation of troubles exists. The objectives of this thesis are : analyse through literature the influence of these surgeries on appearance and disappearance’s process of temporomandibular disorders, study the concerned mechanisms, determine which factors influence the result of these surgical protocols on the patient’s articular condition, determine which prevention methods can be used.
