Complication endothéliale pulmonaire après Chimiothérapie Haute Dose comportant du Busulfan

Abstract : Introduction: The objective of this study was the description of respiratory endothelial toxicities following administration of High Dose Chemotherapy (HDC) containing Busulfan in pediatric oncology department. Methods: All patients under the age of 20 years at diagnosis, treated with HDC containing Busulfan between 2003 and 2015 and followed in Gustave Roussy Pediatric Department of Oncology were analyzed. Results: 220 patients were identified and 214 were included. 35 patients had respiratory symptoms after Busulfan- Melphalan (30 patients) and Busulfan-Thiotepa (5 patients) HDC regimen. At initial diagnosis, 4 patients were suspected of respiratory endothelial toxicity, 10 patients had infectious related symptoms, and 7 patients presented with pulmonary hemorrhage. The nature of the toxicity was undetermined in 14 patients. A second look of all available thoracic Computed Tomography (CT) of the 35 patients led to the identification of 10 patients with a respiratory endothelial toxicity, including 6 additional patients, who were not suspected at initial diagnosis. Conclusion: Respiratory endothelial toxicities are rare. Diagnosis is difficult in these patients, for whom thoracic CT is the most informative and non-invasive exam to guide the diagnosis. Few therapeutic options are available and outcomes are mostly unfavorable. Early detection of patients with endothelial respiratory toxicities is of major importance, in order to offer them adapted treatment and follow-up.
