Prescriptions médicamenteuses inappropriées d'aspirine chez la personne âgée : comment améliorer nos pratiques ?

Abstract : Introduction: in elderly, inappropriate prescribing (IP) of aspirin is very common. However, this treatment is provider of iatrogenic accidents and admissions to emergencies. The objective is to evaluate the relevance of prescriptions of aspirin in elderly patient hospitalized in geriatric unit. Methods: a prospective multicenter study was conducted between March 2017 and April 2018. All patients aged 75 years and older with a prescription for aspirin were included. The prescriptions were re-evaluated in the light of medical data, STOPP / START v2 criteria and guidelines from health authorities and learned societies. Results: 500 patients with a mean age of 87 years were included in the study. A theoretical indication for aspirin treatment was found in 77% of cases. After reassessment, 157 IP (31%) were intercepted. 117 prescriptions (23%) were stopped because of a prescription of aspirin in primary prevention (71%) and a prescription of aspirin for the management of atrial fibrillation (25%). A change in dosage was made for 40 prescriptions (8%). Conclusion: this study highlights the high prevalence of aspirin IP in elderly. Multiprofessional reassessment of prescriptions is a simple and effective solution for detecting inappropriate prescriptions and limiting drug-related adverse events. Promoting tools in hospital, nursing home and ambulatory care is needed to improve prescribing practices.
