L’antibiothérapie en EHPAD vue par les médecins généralistes de Haute-Savoie : les déterminants de cette prescription

Abstract : Background. France is the third largest antibiotic prescribing country in Europe. General practitioners write a large number of these prescriptions during home visits. The patients residing in nursing homes are particularly exposed to the risks of infections and to the inappropriate use of antibiotics. The misuse of antibiotics is responsible for antimicrobial resistance, a major public health issue. Purpose The objective of this study was to identify the factors that general practitioners in Haute-Savoie take into account when prescribing antibiotics in nursing homes. Method. A qualitative study was undertaken using semi-directed individual interviews with general practitioners in Haute-Savoie. The interviews were carried out until data saturation was reached. Data analysis was performed using both open and axial coding and were subjected to triangulation. Results. Antibiotic prescription in nursing homes was adjusted according to internal institution organisation, the distinctive characteristics of the patient group and the doctor’s own personal practice. The liaison physician and the nurses played a key role in antibiotic prescriptions, as well as the family. Doctors often considered their visits to be time-consuming. Although the concept of antimicrobial resistance was known, it remained abstract. To alleviate the difficulties of antibiotic therapy in long-term care facilities, doctors suggested ways to improve the situation. Conclusion. Antibiotic therapy in nursing homes involves many factors. In order to optimise these prescriptions, it is necessary to put in place specific guidelines and to implement a new visiting system to these institutions.
