Les freins à l’abord de l’incontinence urinaire des femmes en médecine générale : étude qualitative auprès de 22 patientes

Abstract : Introduction: female urinary incontinence is a major public health problem not only because of its frequency but also because of its impact on women’s quality of life. However, it is very rarely discussed in general medecine consultation. Objective: the main objective of the study is to identify the barriers to women’s urinary incontinence in general practice. The secondary objective is to improve screening. Material and Methods: this is a qualitative study of 22 semi-directed interviews of patients of all ages with urinary incontinence. Results: women discuss the subject with their general practitionner despite the multiple barriers they face in discussing it. Women feel ashamed and embarassed to talk about it. They trivialize and minimize this issue, which they consider taboo ad intimate. They find that general practitioners lack interest in this subject. They also assign the screening role to gynaecologists. The difficulties are also related to the organization of the health care system. Urinary incontinence is a secondary reason for consultation. The general practitioners do not have enough time to screen it. Medical demography, with the lack of general pratitioners and the presence of gynaecologists and midwives are also responsible for this. Finally, patient’s lack of knowledge and apprehension of therapeutic management prevents them from talking about it. Discussion: informing and sensitizing women and general practitioners to talk about urinary incontinence are ways to improve the screening of urinary incontinence in general pratice.
