Approche de la prévention secondaire chez des individus adultes atteints de pathologie coronarienne : entre acceptation de la maladie et adhésion thérapeutique

Abstract : Introduction: chronical diseases have raised short and long termes political issues. The number of coronarian patients constantly increases despite primary and secondary preventions. Because the patients are reluctant to change their behavior, we suggest that the illness acceptation is linked to the therapeutic adhesion. Methods: the study combines 50 quantitative patients' data, based on 3 surveys (AIS, IPQ, QUILAM) distributed in 3 centers, and 10 qualitative semi-directive interviews. Those interviews were analysed on recurrent themes and statistical analyses were performed. Results: interviews showed 4 dimensions: cured-curer relationship, disease experiences, behaviors changes, and issues from disease. The quantitative analysis showed there were no link between acceptation and adhesion, but showed that the perception of the illness was linked to the patient's adhesion. The lack of specificity of the survey and the fact that we could not perform long interviews during our professional practices brought us to compose a tool to explore the patients' representations in order to increase their therapeutic adhesion. Conclusion: the 3 main points to the adhesion are: the illness perception, the capacity of behaviors changing and the quality of social and professional supports.
