Positionnement des médecins généralistes face aux maladies chroniques de leurs proches. Étude qualitative réalisée en 2018 auprès de treize médecins généralistes installés à la Réunion

Abstract : Introduction: In France, any physician may be designated as a treating physician for his or her relatives. His role is central to the management of chronic diseases. The purpose of this study was to determine how general practitioners position themselves in the face of their loved ones' chronic diseases. Method: 13 general practitioners have been interroged in semi-structured interviews in Reunion island. It was a thematic analysis. Results: Four attitudes were highlighted: 1. Distant attitude; 2. Neutral attitude, role of consultant with occasional interventions if necessary; 3. Attitude involved, as coordinator: the physicians always check the management proposed by the attending physician and complete it. 4. Attending physician (officially or not). Each physician may adopt several attitudes, according to six predominant factors : the physician himself, the relative, their relationship, the management by the attending physician, the chronic disease, and the physical distance. The consequences of these attitudes are relational, medical, and interprofessional. Conclusion: There is no good or wrong answer to a loved one’s care request, each situation is unique. If the doctor chooses to be involved in the care of his relatives, certain precautions are necessary, especially if he has chosen to be their attending physician: impose a framework, have a standardized attitude, recognize his own limitations, and know how to deleguate.
