Prescription de médicaments à service médical rendu insuffisant dans la rhinopharyngite aiguë : une étude qualitative sur les pratiques au cours de l’internat de médecine générale

Abstract : Background: Acute Rhinopharyngitis (AR) is a benign pathology with a spontaneous favorable evolution. However, drug prescriptions remain common with related drugs having an insufficient risk-benefit balance. The main objective of this study was to understand the reasons that can lead a young general practitioner to prescribe these drugs. The secondary objective was to identify alternatives to drug prescriptions. Method: A qualitative study on the practices of young general practitioners in Grenoble, medical interns and locum doctors with less than one year of practice, carried out by means of focused group and individual interviews. Results: A total of 26 young physicians took part in this study, including 11 interns and 15 locums. The most common practice consisted in prescribing paracetamol combined with rhinopharyngeal clearance. Some participants reported prescribing drugs with insufficient therapeutic value (ITV). Their knowledge of these drugs did not come from their medical courses but from their internship supervisor, personal experiences or patient feedback. The main reason for prescribing them was the difficulty in meeting patients’ requests compounded by a lack of confidence. Alternatives to drug prescription involved patient education and reassurance. Conclusion: These results suggest that young practitioners may be driven to ITV drug prescription in case of AR. The most frequent factor was patient pressure. The main solution resides in patient education.
