Nutritions parentérales pédiatriques standard : étude de stabilité de mélanges binaires et ternaires destinés à la néonatologie

Abstract : The instruction of the DGOS of March 20, 2015 relating to the report of the IGAS on pediatric parenteral nutrition recommends the use of standard MPNs as well as the realization of the stability study. The purpose of this work was to determine the stability of standard mixtures. Four local formulas (2 binaries and 2 ternaries) in accordance with the ESPGHAN recommendations were analyzed at different times and until D56 after storage at 4°C with light protection and after 24 hours at room temperature. Several stabilities were studied: galenic stability of lipid emulsion (zeta potential, particle size distribution and visual observation); chemical stability (electrolytes, pH and osmolality, amino acids and vitamins) and sterility assay. Results of the various analyzes identify stability profiles of any elements. As a critical point in the stability of ternary MPNs, lipid emulsions showed stability for 56 days. Only 3 elements (vitamins C and E, methionine) are not stable during the entire study. Despite their degradation, the amounts found at D7 for vitamin C and D28 for vitamin E still cover daily needs of a premature baby. Taking into account the microbiological risk, it would be possible to set the stability of the formulas at 28 days; Y supplementation for vitamin C would be necessary.
