Évaluation de la récidive après lambeau infra-hyoïdien versus lambeau libre dans la reconstruction des exérèses de carcinomes épidermoïdes de la cavité buccale

Abstract : The infrahyoid myocutaneous pedicled flap (IHF) is a simpler and reliable alternative to free flap (FF) reconstruction in the oral cavity. However, because of the risk of involved neck nodes the oncological safety of the IHF remains controversial. Retrospective chart review from 2006 to 2016. All patients had undergone primary resection of oral cavity squamous cell carcinoma with neck dissection and IHF or FF reconstruction. Evaluation of recurrence rates. We identified 239 patients in the IHF group and 98 in the FF group. The overall recurrence rate was 13.8% for IHF group and 32.6% for the FF group (p=0.0257).In the N+ group, recurrence rate was also lower in IHF group than FF group (p=0.0233). Multivariate analysis shows that involved nodes and absence of adjuvant radiotherapy are significantly associated to recurrence. The IHF is oncologically safe in oral cavity squamous cell carcinoma reconstructions including for patients with involved neck nodes.
