Les complications iatrogènes des matériaux à base d'hydroxyde de calcium au cours des soins dentaires : une revue systématique de la littérature

Abstract : Despite a common use, calcium hydroxide (CH) chemical properties give it a potential toxicity and misuse could lead to iatrogenic complications. A systematic review was performed to identify the complication types, management, prognosis and potential influencing factors of CH materials accidents. PubMed, EMBASE, DOSS and Cochrane Library databases were asked, from 1980 to December 2018. Articles that answered the research question, including case reports or series, and clinical studies, in English or French, were included. Suitable data were ranked as pre, intra and postoperative variables. A quality assessment was performed. 28 articles, meting the inclusion criteria, were analyzed, reporting on a total of 56 cases. Almost all included studies were case reports. The main founded complications were apical overextension (42.9%), inferior alveolar canal extrusion (26,8%), soft tissues extrusion (8.9%), maxillary sinus extrusion (7.1%) and intravascular injection (7,1%). Treatments were mainly surgical (44.6%) or abstention (37.5%). Permanent after effects were reported in 19.6%. Within the poverty and low level of evidence of the published data, CH could lead to serious adverse effects. Management is depending on the accident type and symptomatology. The prognosis is mostly favorable, but postoperative dysesthesia appears to be a poorer functional prognosis factor. Tooth locations or root filling technique may be influencing factors.
