Prise en charge médicale des entorses de cheville succédant à un saut en parachute à ouverture automatique : audit clinique rétrospectif de 2013 à 2017 au sein de l'école des troupes aéroportées de Pau

Abstract : Context: Ankle trauma is the most common traumatic pathology in military parachuting. This study evaluates the medical treatment of ankle sprains following an MSLP at the Paratrooper school of Pau (ETAP), France, between January 1st 2013 and January 2017. Method: This study analyzed all ankle injuries following a SOA and then the medical response to ankle sprains after excluding fractures. The medical care offered during the following visits was audited following the refreshed evaluation grid of the fifth consensus conference of Roanne, 1995. Results: Among the 249 traumas, 224 were included. The Ottawa Ankle rules sensitivity reached 100% for a specificity of 27,5% after the first two consultations. The RICE protocol and contention were prescribed over 90% of the cases. Kinesiotherapy was prescribed for 46% of the patients. 50% of the patients were not seen on a follow-up visit. Patients, for whom a severity diagnostic was not performed during the initial consultation, are particularly not followed. The follow-up visit for 64% of serious ankle sprains did not follow recommendations. Conclusion: Military doctors correctly apply the recommendations. However, improvements are needed to optimize the medical care. Two action plans were carried out: one for the physicians working in paratrooper school’s medical unit and one for the doctors engaged in parachute regiments. A reassessment of the improvement actions will be needed to ensure their effectiveness.
