Analyse rétrospective des caractéristiques des patients répondeurs au collyre de sérum autologue

Abstract : Purpose: Autologous serum eye drops (ASED) are used worldwide to treat dry eye disease (DED). However, its biological composition has not been well investigated and predictive factor of effectiveness remains to be identified. Main objective of the study is to analyze the response of patients presenting DED treated by ASED in routine care in relationship with ASED biological composition. Methods: Retrospective observational study conducted in a single center university hospital.50 patients (87 eyes) presenting DED resulting from with various etiologies were included base on an Ocular Surface Disease Index (OSDI) ≥ 20 and a DED refractory to conventional treatment. Each patient used 8 drops a day per treated eyes of 20%diluted ASED until next scheduled follow up. Non diluted serum was used for extensive biological characterization. Subjective and objectives symptoms were recorded before the initiation of ASED and closer to the sixth month of treatment. Responders were defined as presenting an improvement from baseline ≥ 14 points in OSDI and/or ≥ 1 grade in Corneal Fluoresence Staining for all eyes treated. Results: OSDI and Oxford scale were significantly reduced from 68.71 ± 23.24 to 54.77 ± 25.66 (p=0.0001) and 3.15 ± 1.49 to 2.10 ± 1.34 (p<0.0001) respectively. 68% of the patients were responders. Non-responders patients presented significantly higher EGF concentration in serum compared to responders (226.4 ± 112.0 pg/mL vs 153.0 ± 90.4 pg/mL; p=0.017). IL1-β and IFN-γ pro inflammatory cytokines showed a trend to superiority in non-responders group. Conclusions: ASED administration offers a significant clinical improvement in the management of DED. Biological differences observed between responders and non-responders suggest that a better understanding of biological activity of ASED is still required.
