Prise en charge globale du syndrome douloureux régional complexe de l'enfant au CHU-Lenval pédiatrique de Nice

Abstract : Introduction : The complex regional pain syndrome present in children is unknown to most healthcare professionals and there is no known recommendation on how to manage this pathology. This study aims to highlight general treatment methods for children monitored by the Algology team from the University Hospital of Lenval in Nice.
Materials and methods : This study, conducted between 2014 and 2019, is both descriptive and retrospective. A total of 72 charts were analyzed in conjunction with a telephone survey of patients who had ceased having follow-ups for more than a year.
Results : The treatment plan usually consists of a step-2 analgesic together with plasters of lidocaine, and, depending on the DN4, an antiepileptic and / or an antidepressant. Very little morphine or local anesthetic nerve blocks were used, the majority benefited from frequent physiotherapy. Many of the techniques used varied (mirror therapy, motor imagery, ...). More than half had psychological follow-ups. Depending on how things progressed, alternative therapies such as TENS and autohypnosis were proposed. Out of all patients monitored, 72.2 % are now considered cured with an average healing time of 13.9 weeks (with a 50 % survival rate at 12.6 weeks).
Conclusion : In this study, it has been concluded that these children require close monitoring, with at least one Algology appointment and psychological monitoring per month, physiotherapy several times a week. Prospective and multicentric studies should be carried out in order to establish a consensus on a universal protocol, reflected at a national level, which would provide guidance to healthcare professionals looking after the children in question.
