L'expérimentation PAERPA et la place du pharmacien dans ce dispositif

Abstract : The proportion of seniors is expected to increase from 8.5% of the population on January 1, 2007 to 9.4% as of January 1, 2020. Over the age of 75, the risk of loss of independence and dependence is doubled. The number of people around the patient is increasing and care is becoming increasingly complex, which sometimes creates break points in the care pathways of these elderly people. The PAERPA (elderly people in risk of loss of autonomy) project was therefore set up in sixteen French pilotes territories to respond to these breakpoints. One of the major challenges is to go beyond pre-existing approaches to structuring actors' coordination and to decompartmentalize the health, medico-social and social sectors in order to better coordinate the care path of these elderly people. This device puts the elderly person at the heart of the system by proposing a personalized health project for people identified as fragile.
