Étude du niveau d’épuisement professionnel et de l’influence de la charge de travail chez les internes de médecine générale en Aquitaine : étude prospective de 2013 à 2016

Abstract : Objectives: assess the link between the level of burnout and the workload evolution since the February 2015 decree about residents working hours. Study the evolution of these two variables during the residency, according to the commuting time and the internship specialty. Methods: a longitudinal prospective study on a cohort of 205 general practice residents followed from October 2013 to November 2016. The residents who volunteered have completed an online self-assessment questionnaire at the end of each semester. Socio-demographics, working conditions and MBI burnout measure data were collected. Results: the prevalence of high level of burnout ranged from 20% to 29% due to emotional exhaustion and from 15.3% to 27.4% due to emotional exhaustion and depersonalization. The workload and the level of burnout were correlated with a delayed effect. The workload decreased under the February 2015 decree. The commuting time had no effect on this correlation. However, the internship specialty had an impact on burnout, generally higher at Adult Medicine, at the Emergency Department and at the University Hospital. Depersonalization was higher in gynecology and pediatrics. Conclusion: the level of burnout is related to the workload with a delayed effect. Burnout detection and prevention tools, as well as measures to ensure compliance with the February 2015 decree, should be implemented during the residents training.
