Atrophie multi-systématisée et exposition professionnelle aux pesticides : vers une reconnaissance en maladie professionnelle

Abstract : Multiple system atrophy (MSA) is a neurodegenerative disease that is extremely close to Parkinson’s disease (PD) in its clinical presentation and pathophysiology. As for PD, links have been established between MSA risk and occupational exposure to pesticides. However, this disease is not included in the tables of occupational diseases. The aim of this work was to promote recognition of MSA as an occupational disease linked with pesticides. We carried out a transversal study of patients with MSA and PD in two hospitals. The exposure modalities were collected by a specific questionnaire and then analysed by an expert. On the socio-demographic and clinical level, a significant difference was found for the average age at diagnosis (p-value = 0.041) and the average time taken to manage the disease (p-value = 0.001). A comparable proportion of patients with MSA and PD was involved in the preparation (p-value = 0.258) and application (p-value = 0.105) of pesticide treatments. The number of preparation per day (p-value = 0.324), the average number of annual processing (p-value = 0.558) and the annual number of cleaning (p-value = 0.318) and repair (p-value = 0.327) of equipment was similar. No significant difference was found between the two pathologies for exposure to molecules of interest in PD (p-value between 0.059 and 0.938). Despite the low number of staff and a difference in data collection between Bordeaux and Toulouse, we were unable to demonstrate a significant difference between the two pathologies; results rather in favour of our hypothesis. It seems necessary to continue this type of study on a larger scale, with systematised methodologies, in order to be able, in the long term, to recognise MSA as an occupational disease in connection with occupational exposure to pesticides.
