Chirurgie de la cataracte chez les patients monophtalmes : une cohorte rétrospective de 100 patients

Abstract : PURPOSE: to determine the course and outcomes of a cataract surgery in one-eyed patients.
PATIENTS AND METHODES: a retrospective cohort study was conducted in the University Hospital Centre of Nice, service of ophthalmology. We included all the one-eyed patients who had undergone a cataract surgery on the functional eye, between January 2014 and December 2018. A one-eyed patient was defined as having a visual acuity (VA) of less than or equal to 0.1 (logMAR +1) in the worst eye. Data was collected from the non-computerized medical records («paper records») at the Pasteur 2 hospital archives. The variables included were: the socio-demographic factors (gender, age, way of life), past medical history, data of preoperative clinical examination, the surgical course, the post-operative clinical examination and the visual outcomes.
RESULTS: a hundred one-eyed patients were included, of which 48% were men and 52% were women (with a mean age of 74,01 years). The mean preoperative VA was of LogMAR +0,74. Fifty-eight patients were operated (58%) in ambulatory. General anaesthesia was used in 29% of patients and locoregional anaesthesia in 9%. In this study, all the cataract surgeries were performed by phacoemulsification. Five patients (5%) had perioperative complications. Eighty-seven patients had an improvement of the VA in comparison with the preoperative examination. On the last clinical examination, the mean final VA was 0.4 (LogMAR+0,45) and 0.8 (LogMAR+0,13) in patient that presented without preoperative past ocular history. In our study the cataract surgery was never the unique cause responsible for a complete visual loss.
CONCLUSION: results seemed to show a low rate of complications in one-eyed patients. In the most cases, cataract surgery provided a good visual recovery, even when an unfortunate surgical complication occurs.
