Enseignement par la simulation de la prise en charge des urgences vitales de l'enfant pour les internes de pédiatrie : état des lieux national, évaluation des besoins et proposition d'un programme d'enseignement

Abstract : Introduction: simulation-based teaching has proven its efficiency in medical training, especially in the management of life-threatening emergencies. This technique has been partially integrated for several years in the initial training of pediatric residents in Bordeaux. Our objective is to propose a simulation-based teaching program for pediatric residents. Method: a national inventory of simulation-based teaching for pediatric residents was conducted using a questionnaire. The skills of pediatric residents in the management of children's life-threatening emergencies were assessed by two studies: one evaluated the knowledge of basic cardiac arrest resuscitation in all of Bordeaux pediatric residents; the other evaluated the team leadership skills of residents at the end of their studies in an immersive simulation session. Results: all French subdivisions offer simulation training to pediatric residents, but major disparities persist. The knowledge of Bordeaux pediatric residents in the initial management of cardio-respiratory arrest is insufficient. Technical and non-technical skills as a team leader in the management of intra-hospital cardio respiratory arrest were insufficient among last year residents. Conclusion: based on the experience of other subdivisions and the assessed needs, we propose a curriculum that combines theoretical and practical training with several types of simulations adapted to each learning objective over a five-year period.
