Évolution des taux de LDLc et de CRPus avant et après un programme de réadaptation cardiaque ambulatoire au décours d'un syndrome coronaire aigu

Abstract : Importance: The acute coronary syndrome (ACS) has proven to be a real inflammatory disease which has allowed the development of new therapeutic targets. To date, few studies have evaluated the impact of the cardiac rehabilitation (CR) on inflammation. Thus, our main objective is to evaluate LDLc (low density lipoprotein cholesterol) and HSCRP (High sensitivity Protein C reactive) before and after CR after SCA. Design, Setting, and Participants: 102 patients undergoing cardiac rehabilitation after an acute coronary syndrome at the CHU LARIBOISIERE were divided into 4 groups, non-randomized according to their HSCRP (HSCRP<2,0mg/L and HSCRP≥2,0mg/L) andLDLc levels (LDLC<0,7g/L and LDLc ≥ 0,7 g/L). HSCRP and LDLc were collected before and after CR. For each patient, the treatment after CR was compared with the treatment recommended by the European Society of Cardiology. The results were then analyzed statistically and discussed. Results: Our results showed a significant decrease of CRPus of 28% ([0.26-1.13], p = 0.002) and 8% of LDLc ([0.01 - 0.11], p = 0.013 ) in the LDLc group ≥ 0.7g / L. There was also a significant CRPus decrease of 36% ([0.837-2.512], p <0.001) in the CRPus group ≥ 2mg / L. Conclusion: These results allow us to highlight the benefits of rehabilitation on inflammation in the high lipid group (LDLc ≥ 0.7 g / L) and high inflammatory value (CRPus≥2,0mg / L).
